Role Overview

We are seeking full-time Physical Therapists to join a team dedicated to providing personalized, one-on-one physical therapy services in the community.

Responsibilities

• Deliver clinical and autonomous interventions to improve patient outcomes.
• Conduct Geriatric House Calls™ to serve older adults in their homes.
• Collaborate within an interdisciplinary team focused on functional goals.
• Create and modify treatment plans based on patient needs.
• Support patient progress through evidence-based practices.
• Participate in ongoing professional development and continuing education.

Qualifications & Skills

• Valid Physical Therapy license or eligibility in the practice state(s).
• Degree from an accredited physical therapy program.
• Basic computer literacy and current CPR certification.
• Ability to work independently and as part of a team.
