Physical Therapist

About the Role

This position involves providing personalized, one-on-one physical therapy to older adults in their homes or senior living communities. The therapist will conduct evaluations, develop individualized treatment plans, and deliver evidence-based interventions to enhance mobility, independence, and quality of life. The role emphasizes flexibility, autonomy, and building strong patient relationships, with options to incorporate telehealth.

Responsibilities

• Perform evaluations and create tailored care plans
• Deliver therapy sessions supporting wellness and functional independence
• Document patient progress in EMR systems
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging-in-place strategies
• Utilize telehealth and virtual care to extend access and flexibility

Requirements

• Valid Physical Therapy license or eligibility in practice state(s)
• Degree from an accredited PT program
• Proficiency with computers and EMR systems
• Current CPR certification
• Ability to lift up to 50 lbs and assist with patient transfers
• Ability to perform CPR and drive between patient sites
• Compliance with PPE protocols
