About the role

We are seeking a dedicated Physical Therapist to join our healthcare team. As a vital member of our rehabilitation services, you will provide comprehensive physical therapy care while working collaboratively with our multidisciplinary team to deliver optimal patient outcomes.

Responsibilities

  • Conduct thorough patient evaluations and develop individualized treatment plans
  • Implement evidence-based physical therapy interventions and monitor patient progress
  • Document patient assessments, treatment plans, and progress notes with attention to detail
  • Supervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians
  • Establish measurable treatment goals and adjust plans based on patient progress
  • Provide patient and caregiver education regarding treatment plans and home exercise programs
  • Collaborate with referring physicians and other healthcare providers
  • Maintain accurate and timely documentation in compliance with healthcare standards
  • Utilize rehabilitation equipment and techniques effectively
  • Participate in regular team meetings and care coordination discussions

Requirements

  • Graduate degree from a CAPTE-accredited Physical Therapy program
  • Current Minnesota state physical therapy license or eligibility to obtain one
  • Valid CPR certification
  • Strong knowledge of anatomy, physiology, and rehabilitation principles
  • Excellent assessment and diagnostic skills
  • Proven ability to develop and implement effective treatment plans
  • Strong documentation and electronic health record skills
  • Outstanding interpersonal and communication abilities
  • Physical stamina and ability to assist patients with mobility
  • Proven problem-solving and critical thinking skills
  • Ability to work independently and as part of a healthcare team
  • Commitment to continuing education and professional development
  • Experience with diverse patient populations preferred

Additional Information

The salary range begins at $72,000 and increases based on relevant experience. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for discretionary incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
