About the role

We are seeking a dedicated Physical Therapist to join our healthcare team in Eden Prairie.

Responsibilities

  • Conduct thorough patient evaluations and develop individualized treatment plans
  • Implement evidence-based physical therapy interventions and monitor patient progress
  • Document patient assessments, treatment plans, and progress notes with attention to detail
  • Supervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians
  • Establish measurable treatment goals and adjust plans based on patient progress
  • Provide patient and caregiver education regarding treatment plans and home exercise programs
  • Collaborate with referring physicians and other healthcare providers
  • Maintain accurate and timely documentation in compliance with healthcare standards
  • Utilize rehabilitation equipment and techniques effectively
  • Participate in regular team meetings and care coordination discussions

Requirements

  • Graduate degree from a CAPTE-accredited Physical Therapy program
  • Current Minnesota state physical therapy license or eligibility to obtain one
  • Valid CPR certification
  • Strong knowledge of anatomy, physiology, and rehabilitation principles
  • Excellent assessment and diagnostic skills
  • Proven ability to develop and implement effective treatment plans
  • Strong documentation and electronic health record skills
  • Outstanding interpersonal and communication abilities
  • Physical stamina and ability to assist patients with mobility
  • Proven problem-solving and critical thinking skills
  • Ability to work independently and as part of a healthcare team
  • Commitment to continuing education and professional development
  • Experience with diverse patient populations preferred

Pay

  • The salary range begins at $72,000 and increases based on relevant experience.
