Physical Therapist

About the role

We are seeking a skilled and motivated Physical Therapist (PT) to join our rehabilitation team. In this role, you will provide high-quality therapy services to residents on an as-needed basis, helping them improve mobility, relieve pain, and restore function in accordance with individualized care plans.

Responsibilities

• Conduct thorough physical therapy evaluations and develop appropriate treatment plans.
• Deliver evidence-based PT interventions tailored to each resident’s needs and goals.
• Monitor and document patient progress and adjust care plans as necessary.
• Communicate effectively with interdisciplinary team members, residents, and their families.
• Ensure accurate and timely clinical documentation in compliance with facility and regulatory standards.

Requirements

• Current and valid Physical Therapist license in the state of PA.
• Graduate of an accredited Physical Therapy program.
• Previous experience in a skilled nursing or long-term care setting is preferred.
• Strong clinical skills and a passion for geriatric care.
• Ability to work independently and adapt to a dynamic environment.
