Physical Therapist (PT) responsible for evaluation, treatment planning, and delivery of skilled PT services to clients. Ensures timely initiation of services, including screening, evaluation, treatment, goal setting, and family/caregiver education. Documents all services for accurate and timely billing, adhering to organizational guidelines, professional standards, and federal/state compliance. Serves as a patient and rehabilitation advocate, ensuring cross-disciplinary coordination for consistent patient care. Participates in quality assurance and customer service activities.

Responsibilities

  • Develops and implements treatment plans within practice guidelines to achieve optimum functional outcomes for patients.
  • Completes documentation including evaluations, daily notes, progress notes, discharge summaries, and monthly reports as required.
  • Directs and supervises assistants, rehab aides, and students in accordance with state and federal guidelines.
  • Participates in care planning meetings, rehab meetings, and family training to communicate patient progress.
  • Educates staff, healthcare professionals, and third-party payers on the benefits and functional outcomes of intervention.
  • Follows organizational policies and procedures to ensure employee and patient safety.
  • Ensures equipment is in good working order prior to use and reports damaged equipment.
  • Maintains a clean environment and attends continuing education to stay current with clinical skills and techniques.
  • Provides in-service training or educational presentations to enhance clinical and professional knowledge.
  • Maintains appropriate state licensure, certification, and credentialing.
  • Submits billing, payroll, and expense documentation in a timely manner.
  • Makes appropriate referrals to other members of the rehab team.
  • Provides regular screening of residents and reviews restorative care plans quarterly.
  • Updates patient progress verbally or in writing to the Director of Rehab and during unit rounds or care plan meetings.
  • Attends interdisciplinary meetings as required by the rehabilitation department.
  • Acts as a liaison therapist to establish communication with outside school therapists and disseminates information to care meetings (e.g., Plan of Care, clinics).
  • Serves as a clinical instructor for PT student affiliates at the facility.
  • Performs additional duties as assigned by the Director of Rehab or upper management.

Requirements

  • Master’s degree in Physical Therapy (for hires prior to October 25, 2024) from an accredited program by the APTA Commission on Accreditation in Physical Therapy Education.
  • Doctoral degree (DPT) in Physical Therapy (for hires on/after October 25, 2024).
  • Minimum one (1) year of experience with the pediatric population preferred.
  • Minimum one (1) year in a residential/assisted living or skilled nursing facility preferred.
  • Minimum one (1) year of experience with CVA, TBI, modalities, and splinting preferred.

Skills

  • Good interpersonal skills.
  • Ability to read, write, and interpret documents.
  • Effective communication with patients and coworkers.
  • Knowledge of all relevant codes and regulations.
  • Adherence to organizational safety policies and Health Department regulations.

Licenses and Certifications

  • Licensure as required by New York State guidelines.
  • Certification through APTA.

Physical Requirements

  • Prolonged periods of standing, reaching, and walking throughout the workday.
  • Stooping, bending, and lifting items up to 25 pounds routinely; ability to lift up to 100 pounds if necessary to assist with patients.
  • Exposure to adverse indoor/outdoor environmental conditions, bloodborne pathogens, physical hazards (e.g., needle pricks), and chemical hazards.

Factors considered may include site, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget, and internal equity.
