This position showcases your skills in client evaluation and treatment to achieve maximum client independence, optimizing individual functional level and potential. Plan and prepare treatment plans based on evaluation to assist clients in restoring function, relieving pain, and preventing disability while in the comfort and security of their home. Responsibilities

• Evaluate clients to determine functional limitations and develop treatment plans
• Treat clients to restore function, relieve pain, and prevent disability

Requirements

• Current Minnesota Physical Therapy license
• Experience in home health care setting preferred
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team

Qualifications

• Master's degree in Physical Therapy from an accredited program
• Passing score on the National Physical Therapy Examination (NPTE)
