About the Role
If you’re a Physical Therapist who is passionate about helping adults regain mobility, independence, and confidence at home, our Adult Homecare Team is where you belong. At Nascentia Health, you’ll provide skilled, compassionate physical therapy to adults with chronic, acute, or complex conditions, right in the comfort of their homes. You’ll have the time, autonomy, and support to focus on what matters most: patient-centered care that truly makes a difference.
You’ll provide one-on-one care to adult patients in their homes, supported by a collaborative team and a structure designed to promote work-life balance. Home visits are scheduled Monday–Friday, 8:00 AM–4:30 PM, with no on-call requirement, and a one weekend a month rotation. This full-time position offers flexible scheduling.
Responsibilities
- Deliver safe, patient-centered physical therapy interventions in the home.
- Perform comprehensive patient assessments using best clinical practices.
- Develop, implement, and update individualized plans of care.
- Serve as a key member of the interdisciplinary care team, communicating patient progress and care needs.
- Educate patients and caregivers on mobility, safety, and therapeutic exercises.
- Ensure accurate, timely, and clear documentation (including OASIS, within regulatory timeframes).
- Supervise and provide oversight of paraprofessional staff, including home health aides, as related to the patient’s plan of care.
- Maintain compliance with infection control, quality improvement, and regulatory standards.
- Manage a diverse adult caseload while meeting productivity expectations.
- Stay current with clinical best practices and participate in ongoing education and in-services.
Requirements
- Current New York State Physical Therapy license.
- Registered with New York State and the American Physical Therapy Association (as applicable).
- Valid NYS driver’s license and automobile insurance.
- One year of physical therapy experience preferred (home care experience a plus).
- Strong clinical judgment, organization, and ability to work independently.
- Excellent communication and documentation skills.
Physical Requirements
- Ability to sit or stand for long periods during the workday.
- Ability to perform tasks involving physical activity (including but not limited to): pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, and ability to go up and down stairs.
- Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
- Visual/hearing ability sufficient to comprehend written or verbal communication.
- Ability to use telephone and communicate over the phone.
- Frequently uses a laptop computer to enter orders and obtain product information.
Bloodborne Pathogens Exposure: Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids, or tissues.
