About the role

We are seeking Physical Therapists to join a team dedicated to providing Geriatric House Calls™ in a 1:1 setting to older adults in their communities.

Responsibilities

• Deliver clinical, autonomous interventions focused on improving functional outcomes through personalized care
• Contribute to an interdisciplinary team
• Develop treatment plans
• Assess patient needs
• Drive rewarding patient outcomes

Qualifications

• Valid Physical Therapy license in the practice state(s)
• Degree from an accredited program
• Basic computer literacy
• Current CPR certification
