About the role
Seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in private homes or Senior Living Communities, focusing on improving mobility, independence, and quality of life through a patient-centered Geriatric House Calls™ model.
Responsibilities
- Providing evaluations, developing individualized treatment plans, and delivering evidence-based therapy
- Supporting patient wellness, mobility, and functional independence
- Documenting care using EMR systems in compliance with regulations
- Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
- Educating patients, caregivers, and staff to promote safety and aging in place
- Using telehealth and virtual care to enhance accessibility and flexibility
Qualifications and skills
- Valid Physical Therapy license or eligibility in practice state
- Graduation from an accredited Physical Therapy program
- Proficiency with computers and EMR systems
- Current CPR certification
Physical requirements
- Ability to exert medium physical effort, including lifting and assisting patients
- Repositioning patients, standing, walking, climbing stairs, and driving to care locations
- Wearing PPE when necessary
