Physical Therapist

About the role

Seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in private homes or Senior Living Communities, focusing on improving mobility, independence, and quality of life through a patient-centered Geriatric House Calls™ model.

Responsibilities

• Providing evaluations, developing individualized treatment plans, and delivering evidence-based therapy
• Supporting patient wellness, mobility, and functional independence
• Documenting care using EMR systems in compliance with regulations
• Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educating patients, caregivers, and staff to promote safety and aging in place
• Using telehealth and virtual care to enhance accessibility and flexibility

Qualifications and skills

• Valid Physical Therapy license or eligibility in practice state
• Graduation from an accredited Physical Therapy program
• Proficiency with computers and EMR systems
• Current CPR certification

Physical requirements

• Ability to exert medium physical effort, including lifting and assisting patients
• Repositioning patients, standing, walking, climbing stairs, and driving to care locations
• Wearing PPE when necessary
