About the role

Evaluate residents and develop treatment plans that meet their needs; choose activities that help residents carry out daily tasks independently; monitor residents as they perform assigned tasks; observe and assess residents’ progress and prepare detailed reports accordingly; assist in rehabilitating residents disabled by injury, illness, or other conditions; maintain a safe and clean working environment; safeguard resident confidentiality by monitoring confidential information handling; maintain documentation of resident care services through record audits; foster cooperative relationships among healthcare teams and contribute to team efforts.

Requirements

• Graduate of an accredited Physical Therapy Program
• Current New York State PT License
• Experience in long-term care settings preferred
