About the Role

The Physical Therapist develops and implements individualized therapy programs to maximize patients' progress toward functional goals. This includes physical assessment, treatment planning, implementation, education, and communication. The role involves maintaining positive working relationships with health professionals and ancillary staff.

Responsibilities

  • Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
  • Reviews patients’ functional needs and adjusts care plans to ensure optimal patient outcomes.
  • Designs and implements a plan of care based on a thorough assessment of potential development, incorporating participation by significant others when appropriate.
  • Conducts therapy practice within defined standards of care.
  • Directs the assistance of support staff to achieve patients’ functional goals.
  • Communicates effectively with other professionals and support staff to achieve positive patient outcomes.
  • Practices facility and departmental policies, objectives, quality improvement programs, safety, environmental, and infection control standards.
  • Documents evaluations, treatment goals, and plans; regularly updates documentation and maintains prescriptions and signatures.
  • Enhances professional growth and development through participation in educational programs, in-service meetings, and workshops; stays updated on professional literature.

Requirements

  • Knowledge and skills to care for the needs of the Skilled Nursing Facility (SNF) population.
  • Strong problem-solving skills to successfully perform the job.
  • Excellent verbal and written communication skills.
  • Excellent organizational skills and attention to detail.
  • Strong customer service orientation.
  • Ability to communicate effectively and professionally with employees, patients, and family members.
  • Ability to travel as needed to perform job duties.

Qualifications

  • Graduate of an accredited Bachelor’s, Master’s, or Doctorate program in Physical Therapy.
  • Current license from the state Board of Physical Therapy Examiners.
  • Proficiency with all applicable state, federal, and professional standards.

Physical Requirements

  • Extensive standing and walking.
  • Lifting, positioning, pushing, and/or transferring patients.
  • Frequent reaching, stooping, bending, kneeling, and crouching.
