Role Overview: Seeking licensed Physical Therapists to deliver personalized, one-on-one care to older adults in private homes and senior living communities, utilizing the Geriatric House Calls™ model. The position offers flexibility, autonomy, and the chance to build meaningful patient relationships, with options to incorporate telehealth and virtual care for enhanced access and continuity. Key Responsibilities: Provide tailored physical therapy sessions in patients' homes or communitiesPerform evaluations, develop individualized treatment plans, and administer evidence-based therapiesSupport patient mobility, independence, and wellnessDocument care accurately in electronic medical recordsCollaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialistsEducate patients, caregivers, and staff on safety and aging-in-place strategiesUtilize telehealth when appropriate to extend care Qualifications & Skills: Valid Physical Therapy licensure or eligibilityDegree from an accredited PT programProficiency in basic computer and EMR systemsCurrent CPR certification Physical Requirements: Ability to exert moderate effort, assist with patient transfers, perform repositioning, and drive to care locations, while wearing PPE as needed.
