About the Role
A licensed Physical Therapist providing personalized, in-home care to older adults in private residences and senior living communities. The role focuses on improving patients’ mobility, independence, and quality of life through evidence-based treatment within a flexible, clinician-driven schedule.
Responsibilities
- Deliver one-on-one physical therapy sessions in various settings
- Conduct evaluations and develop individualized care plans
- Implement therapy to promote wellness, longevity, and functional independence
- Document patient progress accurately in EMR systems
- Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and staff on safety and aging in place
- Utilize telehealth and virtual care to enhance service delivery
Qualifications & Skills
- Valid Physical Therapy license or eligibility
- Degree from an accredited PT program
- Proficiency in computer use and EMR documentation
- Current CPR certification
Physical Requirements
- Ability to perform moderate physical effort, including lifting and repositioning patients
- Capability to stand, walk, climb stairs, and assist with transfers
- Willingness to drive to patient locations and wear PPE as needed
