Physical Therapist

About the Role

A licensed Physical Therapist providing personalized, in-home care to older adults in private residences and senior living communities. The role focuses on improving patients’ mobility, independence, and quality of life through evidence-based treatment within a flexible, clinician-driven schedule.

Responsibilities

• Deliver one-on-one physical therapy sessions in various settings
• Conduct evaluations and develop individualized care plans
• Implement therapy to promote wellness, longevity, and functional independence
• Document patient progress accurately in EMR systems
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Utilize telehealth and virtual care to enhance service delivery

Qualifications & Skills

• Valid Physical Therapy license or eligibility
• Degree from an accredited PT program
• Proficiency in computer use and EMR documentation
• Current CPR certification

Physical Requirements

• Ability to perform moderate physical effort, including lifting and repositioning patients
• Capability to stand, walk, climb stairs, and assist with transfers
• Willingness to drive to patient locations and wear PPE as needed
