Licensed Physical Therapist providing personalized, in-home and community-based care to older adults, focusing on improving mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations and develop individualized treatment plans
• Deliver evidence-based therapy sessions
• Document care in electronic medical records (EMR)
• Collaborate with interdisciplinary teams
• Educate patients and caregivers on home exercise programs and safety
• Utilize telehealth/virtual care as appropriate

Qualifications

• Valid Physical Therapist license
• Degree from an accredited Physical Therapy program
• Proficiency with EMR and computers
• Current CPR certification
• Physical ability to perform in-home care tasks including lifting and patient assistance

Work environment and schedule
