Physical Therapist

About the role

We are seeking a forward-thinking Physical Therapist who excels at assessing patients’ strengths and weaknesses and devising creative and effective treatment plans. You will practice discretion and maintain patient confidentiality while operating in a high-energy, positive-thinking environment. Your role will involve offering patients positive encouragement to achieve their goals, utilizing your training and skills to aid in healing, recovery, pain improvement, and maximizing functional independence.

As a Staff Physical Therapist, you will examine patients using fundamental clinical knowledge, implement treatment plans, and execute daily care.

Responsibilities

• Collect and record patient care data following the format and guidelines of patient care services, regulatory agencies, and third-party payers, including initial evaluations, daily notes, reassessments, progress reports, discharge plans, and team communications. Record daily billing and follow department productivity guidelines.
• Prepare for evaluations by obtaining patient history, researching diagnoses, selecting and administering tests and measures, and synthesizing results to formulate diagnoses, goals, prognosis, and plans of care.
• Establish and modify treatment plans based on evaluation results, goals, and best practices in collaboration with the interdisciplinary team, patients, and their support systems.
• Assess and reassess the need for plan modifications and refer patients to other healthcare professionals or services as needed.
• Initiate and complete discharge plans, providing education and training to patients and their support systems, including ADLs, IADLs, therapeutic activities, rehabilitation equipment, and community reintegration.
• Provide safe, effective, and compliant interventions, including therapeutic exercises, joint mobilizations, and modalities.
• Document all treatments using Prompt EMR and oversee support staff, including PTAs, ATCs, and PT aides.
• Attend the annual JAG Billing and Coding seminar and promote JAG in the community through lectures and presentations.

Requirements

• Bachelor’s or Master’s Degree required; Doctorate degree preferred.
• Licensed Physical Therapist in the state of practice (NY, NJ).
• Completed state-regulated CEU requirements.
• Valid CPR license.
• Experience or knowledge with EMR software is preferred.
• Excellent oral and written communication skills.
• Strong clinical decision-making skills.
• Dedication to patient success and a strong work ethic.
• Basic computer skills.
• Ability to work both as a team member and independently with minimal supervision.
• Maintain a positive, professional demeanor at all times.
