Physical Therapist

About the role

Join as a licensed Physical Therapist dedicated to delivering personalized, one-on-one care to older adults in their homes, senior living communities, or through virtual platforms.

Responsibilities

• Provide individualized physical therapy to enhance mobility, independence, and quality of life.
• Perform patient evaluations, develop care plans, and implement evidence-based treatments.
• Document patient progress accurately within electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth and virtual care to extend reach and flexibility.

Qualifications

• Valid Physical Therapy license or eligibility in the practice state.
• Degree from an accredited Physical Therapy program.
• Proficiency in computer use and electronic medical records.
• Current CPR certification.

Physical requirements

• Ability to perform moderate physical effort.
• Assist with patient transfers.
• Stand and walk for extended periods.
• Drive to patient locations.
• Wear PPE as needed.
