Creekside Transitional Care & Rehab in Meridian, ID is a post-acute care facility offering long-term care, short-term care, and geriatric services. Join our stable and fun in-house therapy team and work with excellent programs serving our geriatric clientele in their recovery journey.

About the role

We are seeking a Full Time Physical Therapist for our in-house rehab program. Open to PTs at all experience levels, including new graduates.

Responsibilities

  • Screen and evaluate patients with cognitive and/or ADL physical functioning disorders, developing appropriate plans of care following all regulatory and clinical practice standards.
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Provide rehabilitative, skilled, and medically necessary treatment interventions per regulatory and clinical practice requirements.
  • Administer procedures as part of the rehabilitation plan, including but not limited to: ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive/assistive devices.
  • Assess patient progress and adjust treatments accordingly in the clinical record.
  • Evaluate treatment outcomes for effectiveness and modify services or make referrals as needed.
  • Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
  • Communicate effectively with supervisors and health team members about patient progress, barriers, and treatment plans.
  • Provide comprehensive discharge summaries of services per regulatory and clinical requirements.

Requirements

  • Physical Therapist license is required.
