About the role

Enhance Rehabilitation is seeking qualified Physical Therapists to join a dynamic team providing the highest quality care for patients in a skilled nursing facility (SNF) setting.

Responsibilities

• Evaluate and treat patients following an individualized plan of care.
• Provide excellent care for patients.
• Provide excellent customer service to patients and family members.
• Collaborate with facility staff to ensure excellent care for patients.
• Evaluate new patients, develop plans of care, and set measurable, functional goals.
• Treat patients, answer their concerns, and educate them on injury, progress, and probable outcomes.
• Supervise Physical Therapist Assistants.
• Re‑evaluate patient progress, revise plans of care, and reset goals.
• Complete timely and accurate documentation, including evaluations, re‑evaluations, discharges, and daily progress notes.
