Enhance Rehabilitation is currently seeking qualified Physical Therapists candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting.

Responsibilities

• Evaluate and treat patients following an individualized plan of care
• Provide excellent care for our patients
• Provide excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluate new patients, developing plans of care and setting measurable, functional goals
• Treat patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervise Physical Therapist Assistants
• Re-evaluate patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including: evaluations, re-evaluations, discharges and daily progress notes
