Responsibilities
- Evaluate all new patients.
- Establish a thorough physical therapy treatment plan based on each patient's individual needs.
- Discuss treatment with patients and record patient conditions after each visit.
- Make alterations and adjustments to establish a physical therapy treatment plan as needed.
- Ensure the most successful outcome for the patient by performing an evaluation, explaining the diagnosis and plan of care, scheduling patients appropriately, modifying plan of care as needed, implementing the best known treatment, and ending with a satisfactory outcome according to guidelines.
- Ensure that all records of discharge patients are complete and include: Each progress note signed.
- Communicate with a referring physician via letter regarding patient status on every follow up and upon discharge.
- Act as EPT ambassador for chosen groups and do quarterly community outreach events.
- Help build and retain a full caseload according to your expected commitment of patient visits per week.
- Advocate for patient’s continuum of care: POC visits, bi-annual and annual PT visits.
- Provide support to all other EPT team members.
- Adhere to all EPT state and federal physical therapy standards, guidelines, and laws.
- Adhere to EPT core values.
Qualifications
- Masters/Doctorate degree in Physical Therapy from an accredited program.
- Current PT Licensure or eligibility to test for PT license required.
