Physical Therapist

About the role

Seeking a licensed Physical Therapist to provide personalized, in-home and community-based care to older adults using a flexible, patient-centered approach.

Responsibilities

• Deliver one-on-one therapy in private residences and senior living settings
• Perform evaluations, create care plans, and administer evidence-based treatments
• Support patient mobility, independence, and wellness through therapy sessions
• Document care accurately in electronic medical records
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Incorporate telehealth/virtual care to enhance accessibility and flexibility

Qualifications and Skills

• Valid Physical Therapy license or eligibility in practice state(s)
• Degree from an accredited program
• Proficiency in computer and EMR systems
• Current CPR certification

Physical Requirements

• Ability to exert medium effort, lift up to 50 lbs occasionally, and assist with patient transfers
• Repositioning, standing, walking, climbing stairs, and using PPE as needed
