About the role
We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in homes and senior communities.
Responsibilities
- Performing evaluations
- Developing individualized treatment plans
- Delivering evidence-based therapy to support mobility, independence, and wellness
- Documenting care in our EMR system
- Collaborating with interdisciplinary teams
- Educating patients, caregivers, and staff to promote safety and aging in place

Qualifications
- Valid PT license
- Degree from an accredited program
- Computer proficiency
- CPR certification
- Ability to lift, reposition, and drive to patient locations
- PPE use when necessary
