Physical Therapist

Seeking a dedicated Full-Time Physical Therapist to provide personalized home health care, focusing on restoring strength, improving mobility, and enhancing quality of life for patients at home. Deliver skilled physical therapy based on individualized treatment plans developed with referring physicians.Assess patient needs, set rehabilitation goals, and adjust techniques to optimize outcomes.Collaborate with interdisciplinary teams including nurses, occupational therapists, speech therapists, and social workers for coordinated care.Educate patients and caregivers on exercises, safety, mobility strategies, and assistive device use.Accurately document visits, assessments, and progress notes.Support patient independence and overall well-being in a home setting. Qualifications include a Master’s degree in Physical Therapy, current licensure in IN, CPR certification, strong communication and problem-solving skills, and reliable transportation. Preferred experience includes at least one year in hospital, home health, or outpatient rehab, with expertise in geriatrics, orthopedics, neurologic rehab, or post-acute care.
