About the role
Licensed Physical Therapist to deliver one-on-one in-home, senior living, and virtual care to older adults across community settings.
Responsibilities
- Conduct comprehensive evaluations and assessments
- Develop and implement individualized, evidence-based treatment plans to improve mobility and independence
- Deliver direct therapy interventions in patients’ homes, senior living communities, or via telehealth
- Document all care encounters in electronic medical records (EMR)
- Collaborate with interdisciplinary teams (e.g., physicians, nurses, caregivers) to coordinate care
- Educate patients and caregivers on safety strategies, fall prevention, and aging-in-place techniques
Requirements
- Valid, unrestricted Physical Therapist license in the state of practice
- Graduate of an accredited Physical Therapy program
- Proficiency with EMR systems and general computer applications
- Current CPR certification
Physical demands
- Ability to lift up to 35 lbs independently; assist with transfers and repositioning as needed
- Prolonged periods of standing, walking, and stair climbing during patient visits
- Occasional driving between patient locations
