Physical Therapist

About the role

Licensed Physical Therapist to deliver one-on-one in-home, senior living, and virtual care to older adults across community settings.

Responsibilities

• Conduct comprehensive evaluations and assessments
• Develop and implement individualized, evidence-based treatment plans to improve mobility and independence
• Deliver direct therapy interventions in patients’ homes, senior living communities, or via telehealth
• Document all care encounters in electronic medical records (EMR)
• Collaborate with interdisciplinary teams (e.g., physicians, nurses, caregivers) to coordinate care
• Educate patients and caregivers on safety strategies, fall prevention, and aging-in-place techniques

Requirements

• Valid, unrestricted Physical Therapist license in the state of practice
• Graduate of an accredited Physical Therapy program
• Proficiency with EMR systems and general computer applications
• Current CPR certification

Physical demands

• Ability to lift up to 35 lbs independently; assist with transfers and repositioning as needed
• Prolonged periods of standing, walking, and stair climbing during patient visits
• Occasional driving between patient locations
