Physical Therapist

About the role

We are seeking a licensed Physical Therapist to provide personalized, in-home and senior living care to older adults, focusing on improving mobility, independence, and quality of life through evidence-based treatments.

Responsibilities

• Offer one-on-one therapy sessions in private residences and senior communities
• Perform evaluations and develop individualized care plans
• Deliver therapy to promote wellness, functional independence, and longevity
• Document care accurately within electronic medical records
• Collaborate with interdisciplinary teams, including PTs, OTs, SLPs, and fitness professionals
• Educate patients, caregivers, and staff on safety, function, and aging in place
• Utilize telehealth and virtual care to enhance access and flexibility

Qualifications and Skills

• Valid Physical Therapy license and degree from an accredited program
• Proficiency with computers and EMR systems
• Current CPR certification
• Ability to meet physical demands of community-based care, including lifting, repositioning, and extended walking
