Physical Therapist

About the role

We are seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in various settings, including private homes and senior living communities. This role offers the chance to positively impact community health, promote aging in place, and enjoy autonomy in your practice within a supportive setting.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to enhance mobility, independence, and quality of life.
• Document progress accurately in electronic medical records in compliance with regulations.
• Collaborate with an interdisciplinary team of healthcare professionals.
• Educate patients, caregivers, and staff on safety, function, and aging in place.
• Utilize telehealth and virtual care to extend access and flexibility.

Requirements

• Valid Physical Therapy license or eligibility, from an accredited program.
• Proficiency in basic computer and EMR systems.
• Current CPR certification.
• Ability to meet physical demands, including lifting, repositioning, and assisting patients, and driving to patient locations.
