Physical Therapist

About the role

Provide personalized in-home, senior-living, and virtual physical-therapy care to older adults, emphasizing mobility, independence, and quality of life.

Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.
• Deliver evidence-based therapeutic interventions to improve functional outcomes.
• Document patient progress and treatment notes in the electronic medical record (EMR).
• Collaborate with an interdisciplinary care team to coordinate holistic patient care.
• Educate patients, caregivers, and staff on safety, fall prevention, and aging-in-place strategies.
• Incorporate telehealth modalities when appropriate to enhance care flexibility and access.
• Perform one-on-one therapy in homes, senior-living communities, and virtual settings.
• Support patient wellness and functional independence through ongoing assessment and intervention.

Requirements

• Valid state-issued Physical Therapist license.
• Degree from an accredited physical-therapy program.
• Proficiency with computer systems and EMR documentation.
• Current CPR certification.
• Ability to meet physical demands, including lifting, repositioning patients, and performing therapeutic activities.
