About the role
Provide personalized in-home, senior-living, and virtual physical-therapy care to older adults, emphasizing mobility, independence, and quality of life.
Responsibilities
- Conduct comprehensive patient evaluations and develop individualized treatment plans.
- Deliver evidence-based therapeutic interventions to improve functional outcomes.
- Document patient progress and treatment notes in the electronic medical record (EMR).
- Collaborate with an interdisciplinary care team to coordinate holistic patient care.
- Educate patients, caregivers, and staff on safety, fall prevention, and aging-in-place strategies.
- Incorporate telehealth modalities when appropriate to enhance care flexibility and access.
- Perform one-on-one therapy in homes, senior-living communities, and virtual settings.
- Support patient wellness and functional independence through ongoing assessment and intervention.
Requirements
- Valid state-issued Physical Therapist license.
- Degree from an accredited physical-therapy program.
- Proficiency with computer systems and EMR documentation.
- Current CPR certification.
- Ability to meet physical demands, including lifting, repositioning patients, and performing therapeutic activities.
