About the role
Join as a Physical Therapist to deliver personalized, in-home and community-based care to older adults.
Responsibilities
- Provide one-on-one physical therapy in private residences and senior living communities.
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy.
- Support patient wellness, mobility, independence, and safety.
- Document care accurately within electronic medical records.
- Collaborate with an interdisciplinary team of healthcare professionals.
- Educate patients, caregivers, and staff to promote aging in place.
- Utilize telehealth and virtual care to enhance access and flexibility.
Qualifications & Skills
- Valid Physical Therapy license or eligibility.
- Degree from an accredited program.
- Proficiency in basic computer use and EMR systems.
- Current CPR certification.
Physical Requirements
- Ability to exert moderate physical effort, including lifting up to 50 lbs.
- Assistance with patient transfers, repositioning, and mobility activities.
- Driving to patient locations and performing therapeutic activities.
