About the role

We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities using our Geriatric House Calls™ model.

Responsibilities

• Conduct evaluations and develop individualized treatment plans
• Deliver evidence-based therapy to enhance mobility and independence
• Document care in an electronic medical record (EMR)
• Collaborate with interdisciplinary teams
• Educate patients and caregivers
• Utilize telehealth when appropriate

Requirements

• Valid Physical Therapist license
• Degree from an accredited Physical Therapy program
• Proficiency in computer and EMR systems
• Current CPR certification

Physical demands

• Lifting and repositioning patients
• Extended periods of walking
• Driving to patient locations
