About the role
Licensed Physical Therapist providing personalized in-home and senior living care, including virtual sessions, to older adults.
Responsibilities
- Conduct evaluations and develop individualized treatment plans
- Deliver evidence-based therapy to improve mobility and independence
- Document care in electronic medical records (EMR)
- Collaborate with interdisciplinary teams
- Educate patients and caregivers on home exercise programs and safety
- Incorporate telehealth when appropriate
Qualifications & Skills
- Valid Physical Therapist license
- Degree from an accredited Physical Therapy program
- Proficiency in EMR and basic computer skills
- Current CPR certification
- Ability to meet physical demands such as lifting and assisting patients
