Physical Therapist

About the role

We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in private homes and senior living communities, utilizing our Geriatric House Calls™ model. This role emphasizes autonomy, meaningful patient relationships, and interdisciplinary collaboration within a supportive, clinician-driven culture. Telehealth and virtual care are incorporated for increased flexibility.

Responsibilities

• Conduct evaluations of older adults in home and senior living settings.
• Develop personalized treatment plans based on patient needs.
• Deliver evidence-based therapy sessions.
• Document patient progress in EMR systems.
• Collaborate with interdisciplinary teams to coordinate care.
• Educate patients, caregivers, and staff to promote safety and independence.

Requirements

• Valid Physical Therapist license.
• Degree from an accredited Physical Therapy program.
• Basic computer skills for EMR documentation.
• CPR certification.

Physical demands

• Lifting and repositioning patients.
• Standing, walking, and climbing stairs.
• Driving to patient locations.
