Physical Therapist

About the role

Blue Stone Therapy is seeking a temporary Physical Therapist (PT) (up to full-time hours to cover a leave) to provide rehabilitation services to adult and geriatric patients at Green Pine Acres and Heritage Living Centers in Menagha and Park Rapids, MN. This role will ultimately settle into a PRN position. The focus is on evaluating and treating patients in a skilled nursing and post-acute rehabilitation setting, improving gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled physical therapy treatment focused on functional and measurable outcomes
• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning
• Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards
• Maintain appropriate patient schedules per established guidelines
• Complete timely, accurate documentation to support quality care and reimbursement requirements
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes

Requirements

• Graduate of an accredited Physical Therapy program
• Current Physical Therapist license (or eligibility for licensure) in the applicable state
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
• Strong clinical reasoning, communication, and organizational skills
• Commitment to patient-centered care and professional standards
