Physical Therapist

About the Role

Blue Ridge Care, a mission‑driven not‑for‑profit organization, is seeking a passionate Physical Therapist to join its PACE (Program of All‑Inclusive Care for the Elderly) team. In this role you will help older adults maintain independence, improve mobility, prevent functional decline, and continue living safely in their communities.

Responsibilities

• Evaluate participants’ strength, mobility, balance, and functional abilities.
• Develop individualized treatment plans that promote independence and safety.
• Provide therapy services in the PACE Center, participants’ homes, and community settings.
• Collaborate with physicians, nurses, social workers, occupational therapists, and other professionals to create holistic care plans.
• Educate participants and caregivers on mobility, exercise, fall prevention, and adaptive equipment.
• Recommend and evaluate durable medical equipment, orthotics, prosthetics, and assistive devices.
• Supervise and mentor Physical Therapy Assistants and rehabilitation staff.
• Support quality‑improvement initiatives that enhance participant outcomes.
• Help seniors achieve their highest level of function and quality of life.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Preferred: Experience working with frail older adults or medically complex populations.
• Preferred: Experience in an interdisciplinary healthcare environment.
• Strong clinical assessment, treatment planning, and communication skills.
• Knowledge of rehabilitation practices, adaptive equipment, mobility training, and fall prevention.
