About the role

Winter Haven Hospital, part of BayCare Health System, is seeking a compassionate and skilled Outpatient Physical Therapist to join their growing rehabilitation team.

Responsibilities

• Evaluate, plan, and deliver evidence-based physical therapy services to adult and geriatric patients in an outpatient setting.
• Collaborate closely with physicians, clinical staff, and interdisciplinary team members to restore function, improve mobility, and enhance quality of life for the patients we serve.
• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Provide therapeutic interventions including exercise prescription, manual therapy, neuromuscular re-education, and functional training.
• Monitor patient progress and adjust treatment plans based on clinical outcomes.
• Accurately document evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory and BayCare standards.
• Collaborate with physicians, case managers, and other healthcare professionals to ensure coordinated care.
• Maintain compliance with all state, federal, and organizational policies and procedures.
• Participate in quality improvement initiatives and continuing education opportunities.
• Support a positive, team-oriented culture aligned with BayCare values.

Requirements

• Required BLS (Basic Life Support)
• Required PT (Physical Therapy) or PT (Physical Therapy) Eligible to obtain PT (Physical Therapy)
• Required Bachelor's Physical Therapy
• PREFERRED 1 year Physical Therapist Experience
• Temporary and licensure within 6 months Required

Qualifications

• Preferred 1 year Physical Therapist Experience

• Excellent communication skills
• Strong clinical judgment and problem-solving abilities
• Ability to work independently and as part of a team
