Physical Therapist

About the role

A Physical Therapist provides personalized, one-on-one care to older adults in their homes or senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments.

Responsibilities

• Conduct evaluations and develop individualized care plans
• Deliver therapy sessions supporting wellness and functional independence
• Document care accurately in EMR systems
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging in place
• Utilize telehealth and virtual care to enhance access and flexibility

Qualifications & Skills

• Valid Physical Therapy license or eligibility
• Degree from an accredited program
• Proficiency in computers and EMR systems
• Current CPR certification

Physical Requirements

• Ability to perform moderate physical effort
• Assist with patient transfers and reposition patients
• Drive between locations while wearing PPE when necessary
