Licensed Physical Therapist providing personalized, in-home and senior living care to older adults, utilizing a flexible schedule and virtual care options.

Responsibilities

• Conduct evaluations
• Develop individualized treatment plans
• Deliver evidence-based therapy to promote mobility, independence, and wellness
• Document patient progress in EMR
• Collaborate with an interdisciplinary team
• Educate patients and caregivers
• Incorporate telehealth when appropriate

Qualifications

• Valid PT license
• Degree from an accredited program
• Proficiency with EMR and basic computer skills
• Current CPR certification
• Ability to meet physical demands such as lifting and patient assistance

Key duties

• Provide one-on-one therapy in various settings
• Support aging in place
