Role Overview This position involves providing direct patient care activities within a home environment, including assessment, treatment planning, implementation, and supervision of physical therapy practices. The role requires instructing patients and families on physical activities, assistive device usage, and treatment procedures. Re-evaluating patients and adjusting treatment plans to maximize benefits is essential, along with collaborating with physicians and other practitioners to integrate care. The role also includes evaluating and fitting prosthetic and orthotic devices and recommending modifications.
