Physical Therapist

About the role

Licensed Physical Therapist providing personalized in-home, senior living, and virtual care to older adults, focusing on improving mobility, independence, and quality of life.

Responsibilities

• Deliver one-on-one therapy sessions in private residences and senior communities.
• Conduct evaluations, develop individualized care plans, and implement evidence-based treatments.
• Support patient wellness, mobility, and functional independence.
• Document care accurately within electronic medical records.
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care to extend access and flexibility.

Qualifications

• Valid Physical Therapy license or eligibility in practice state(s).
• Degree from an accredited PT program.
• Proficiency in basic computer and EMR systems.
• Current CPR certification.

Physical Requirements

• Ability to perform moderate physical effort.
• Assist with patient transfers.
• Reposition patients.
• Drive to patient locations.
