About the Role
The Physical Therapist in Home Health evaluates patients' functional status, mobility, endurance, and pain to develop personalized treatment plans.
Responsibilities
- Assess patient needs and assist in creating therapy plans in collaboration with physicians
- Provide treatments within legal and professional scope to improve or maintain patient function
- Monitor, observe, and report patient responses and condition changes to the healthcare team
- Educate patients, families, and team members about care plans, limitations, and progress
- Visit patients as per the plan, document progress, and develop care strategies adhering to state guidelines
- Report evaluation outcomes, goals, and care frequency estimates
- Ensure ability to work effectively in diverse home environments regardless of patient background
Requirements
- Current Physical Therapy license in the practice state
- Valid driver’s license, vehicle insurance, and access to reliable transportation
Preferred Qualifications
- CPR certification or ability to obtain within 90 days
