About the role

Licensed Physical Therapist delivering personalized in-home and senior living care to older adults, focusing on improving mobility, independence, and quality of life.

Responsibilities

• Provide one-on-one therapy in private residences and senior communities
• Perform evaluations, develop care plans, and deliver evidence-based treatments
• Support patient wellness, functional independence, and longevity
• Document care using EMR systems in compliance with regulations
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Utilize telehealth and virtual care to enhance access and flexibility

Qualifications & Skills

• Valid Physical Therapy license or eligibility
• Degree from an accredited program
• Proficiency in computer use and EMR systems
• Current CPR certification

Physical Requirements

• Ability to lift up to 50 lbs
• Assist with patient transfers
• Stand/walk for extended periods
• Climb stairs
• Drive between locations
• PPE usage as needed
