Physical Therapist

About the Role

Licensed Physical Therapist providing one-on-one care to older adults in private homes and senior living communities, utilizing the Geriatric House Calls™ model.

Responsibilities

• Conduct evaluations, develop care plans, and deliver evidence-based treatments to support mobility, independence, and wellness.
• Document care within EMR systems and collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Incorporate telehealth and virtual care to enhance access and continuity.

Requirements

• Valid PT license and degree from an accredited program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.
• Ability to meet physical demands such as lifting, repositioning patients, and performing therapeutic activities.
