Physical Therapist

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide skilled physical therapy services focused on mobility, balance, strength, and independence
• Accurately document evaluations, treatments, and progress in Net Health and PCC
• Collaborate with nursing, therapy, and interdisciplinary teams
• Maintain compliance with all facility and regulatory guidelines

Qualifications

• Current Ohio PT license
• SNF/LTC experience preferred but not required
• Experience with Net Health and PCC preferred
• Strong communication and clinical skills
• Compassionate, dependable, and team-focused mindset
