About the role
Join as a licensed Physical Therapist delivering personalized in-home and senior living care to older adults.
Responsibilities
- Provide one-on-one physical therapy sessions across various settings, including private residences and senior communities
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapies to enhance mobility, independence, and quality of life
- Document patient progress accurately within EMR systems
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging-in-place strategies
- Incorporate telehealth and virtual care to extend access and flexibility
Qualifications and Skills Required
- Valid Physical Therapy license in applicable state(s) or eligibility
- Degree from an accredited PT program
- Proficiency with computers and EMR systems
- Current CPR certification
Physical demands: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to various locations. PPE may be required.
