Physical Therapist

Responsibilities

• Provide compassionate, evidence-based rehabilitation services to seniors and medically complex residents.
• Assess residents' functional status.
• Design and implement individualized treatment plans.
• Deliver interventions to enhance strength, mobility, balance, and safety.
• Collaborate with an interdisciplinary team.
• Educate residents and families about care plans.
• Document progress and outcomes.
• Support safe discharges and transition of care.

Qualifications & Skills

• Current licensure as a Physical Therapist.
• Strong assessment and treatment planning skills.
• Excellent communication and collaboration abilities.
• Ability to work compassionately with elderly and medically complex populations.
• Knowledge of resident-centered, dignity-focused care practices.
