Physical Therapist

About the role

Licensed Physical Therapist responsible for providing personalized, one-on-one care to older adults in private homes and senior living communities, supporting mobility, independence, and quality of life.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy
• Support patient wellness, functional independence, and longevity through therapy sessions
• Document patient progress accurately using electronic medical records
• Collaborate with interdisciplinary teams including OT, SLP, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Utilize telehealth and virtual care to extend access and flexibility

Qualifications & Skills

• Valid Physical Therapy license or eligibility in the practice state
• Degree from an accredited Physical Therapy program
• Proficiency with computers and EMR systems
• Current CPR certification

Physical Requirements

• Ability to exert moderate effort, assist with patient transfers, perform CPR, and drive between locations
• Physical demands include lifting, repositioning patients, and extended walking or standing
