Physical Therapist

About the role

We are seeking licensed Physical Therapists to provide personalized, one-on-one care to older adults in their homes or senior living communities, utilizing our Geriatric House Calls™ model. The role offers autonomy, flexibility, and the opportunity to build meaningful patient relationships, with options to incorporate telehealth for enhanced care delivery.

Responsibilities

• Deliver evidence-based physical therapy to promote mobility, independence, and overall wellness
• Conduct evaluations, develop individualized treatment plans, and document progress accurately
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to support safety and aging in place
• Utilize telehealth/virtual care to extend access and flexibility

Qualifications

• Valid Physical Therapy license or eligibility in the practice state
• Degree from an accredited PT program
• Proficiency in computer use and EMR documentation
• Current CPR certification
• Ability to perform physical tasks including lifting, repositioning, and patient assistance
