Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide physical therapy treatments and therapeutic exercises
• Monitor residents’ progress and adjust treatment plans as needed
• Collaborate with nursing and other healthcare professionals
• Maintain accurate and timely clinical documentation
• Help residents improve mobility, strength, independence, and quality of life

Requirements

• Active Physical Therapist license in the State of New Jersey – Required
• Graduate of an accredited Physical Therapy program
• Strong communication and interpersonal skills
• Compassionate and resident-focused approach
• Skilled Nursing/LTC experience preferred
