Physical Therapist Assistant Senior Care Coordinator (51377)

Responsibilities

• Aid in the implementation of physical therapy interventions under the supervision of a Physical Therapist (PT).
• Work with residents to enhance their mobility, strength, and physical function.
• Support residents in recovering from injuries, surgeries, or illnesses through physical therapy exercises and techniques.
• Help residents improve their balance, coordination, and functional mobility to achieve greater independence.
• Maintain accurate records of treatments, progress, and documentation to comply with healthcare regulations.
• Collaborate with the multidisciplinary care team, including physical therapists, occupational therapists, nurses, and speech therapists, to ensure comprehensive care.
• Participate in care planning meetings to review resident progress and adjust therapy goals.
• Educate residents and their families on mobility strategies, injury prevention, and rehabilitation exercises.

Requirements

• Graduate of an accredited school with an associate degree as a Physical Therapy Assistant.
• Certification by the State of Wisconsin to practice as a Physical Therapy Assistant.
