Physical Therapist Assistant - Home Visits

About the role

We are seeking a licensed Physical Therapist Assistant to deliver patient-centered care in a home health setting.

Responsibilities

• Follow state laws and supervising physical therapists' treatment plans
• Instruct and assist patients with exercises, gait, transfers, and safety training
• Monitor and document patient responses and communicate with healthcare team members
• Educate patients, families, and caregivers on therapy techniques within scope

Requirements

• Current license or certification in physical therapy assistance
• CPR certification
• Valid driver’s license, access to reliable transportation, and ability to work in diverse home environments
• Willingness to work weekend rotations

Preferred skills

• Physical ability to perform patient mobility tasks such as lifting and transferring
